A cross-sectional analysis was performed to determine whether cerebrospinal fluid (CSF) biomarkers could be used to predict response to antiretroviral therapy (ART) in patients with HIV-associated central nervous system disease. A cross-sectional study of 61 patients with HIV-associated central nervous system disease (21 with AIDS) treated with ART at The University of Mississippi Medical Center from January 2001 to December 2005 was performed. Patients with available CSF concentrations of beta-2-microglobulin (beta2-MG), neopterin, neopterin-d4, and human immunodeficiency virus (HIV) RNA were included in the analysis. The mean (SD) age of the patients was 42.9 (8.0) years, the majority (71%) was black, and the median CD4 cell count was 63 (19) cells/mm(3). The mean (SD) duration of ART was 5.8 (2.4) years. Logistic regression analysis was used to identify predictors of virologic suppression. The median log10 viral load was 4.7, and 54% of patients were virologically suppressed. The mean (SD) concentration of beta2-MG was 148 (84) mg/L, and the median concentration of neopterin was 30 (88) nmol/L. The median neopterin-d4 concentration was 4.7 (11) nmol/L. Virologic suppression was associated with a lower neopterin-d4 concentration (P =.04), a lower concentration of beta2-MG (P =.03), and a higher concentration of neopterin (P =.01) but was not associated with neopterin-d4 to neopterin ratio.
